I usually just use the AIA G702-703 payment application form. On the 703 page, I break down the job into whatever chunks seem appropriate: Service, Power (branch wiring), Lighting, Trim, etc. With a little web searching, you can find a downloadable Excel file.

Other than all of the "helpful" notes you have to delete, this is pretty complete.

Attached is link to an excel file that is a sample schedule of values from one of our previous projects. It was just built for internal use, but hopefully it's pretty self-explanatory and something you can adapt for your own use.
