take a look at the OSHA web site www.osha.gov, It has a lot of information that covers this.
But most plants and work site have a safety coordinator who has the companys safety policy that should be followed.
You shouls see if the company already has the written lock out tag out procedure.

OSHA electrical specifications are in NFPA 70E Electrical Safety requirements for employee workplaces. It has LO/TO procedures. The companies that sell LO/TO equipment have sample procedures and training programs they can provide you.
