Should the grounding electrode conductor that connects a ground rod or triad ground system to the main service ground be installed in conduit when routed below grade OR should the conductor (assuming it is bare) be routed so that it is in contact with the earth? If in conduit, does it matter if the conductor is covered, insulated or bare?

The only thing you need to do for the grounding electrode conductor is protect it from physical damage. This usually just means sleeving it in a piece of conduit where it runs up the wall from the ground. You can then bury it or sleeve it in conduit, whatever you think is best. 250.62 lists all the permitted conductor properties.

Make sure that if you do install the Grounding Electrode Conductor is a sleeve that you conform with 250.64 (E) 2002 and 2005 code
