277 volt light fixture mounting limitations

I seem to remember a requirement that 277V fixtures be mounted at least 8' A.F.F. but I can not find this in the 2002 code. Does anyone remember this? Was it an old requirement??

Also, while looking I came across the requirement that 277V fixtures be 3' from doors, windows, handrails etc. Does anybody know the intent here. Would having the fixtures within 3' of non-operable windows be ok. Is the idea to keep them away from metal objects that could create a path to ground or to keep them away from people all together. If the latter, then why not mandate the minimum mounting height?

Not an NEC requirement.
The city of Chicago has their own code, and an 8' height req.
