Is there a part of the code (setting common sence aside) the provides the requirements for the spacing of pull boxes in conduit run in ground. Example would be conduit run to a light pole or an access gate for a CCTV camera or card reader?

No maximum distance, however with limited energy and Chapter 8 applications such as CCTV, the cable can be damaged if the pulling tension is too high. Some raceways are easier to pull thru than others, example fiberglass or HDPE is very smooth, where as PVC conduit with PVC cable is a much harder pull.
And of course if you coax and wire comes on 500 ft spools, then your pull boxes will be perhaps 450 feet apart.
The maxium distance is more a function of the number of bends, cable type and conduit type.

I hope you have a strong vacuum and pull string.Greenlee makes a tension guage for their pulling machine. The wire you are pulling 500 feet or so should have a tension rating from the manufacturer.
Use the lube.
