In my opinion, 90% of all installations won't benefit from the use of isolated grounds.
But to answer your question, an insulated isolated grounding conductor originating at either the service or brancg circuit panelboard will connect directly to the terminal on the IG receptacle. Another equipment grounding conductor will run with the other circuit conductors and the IG conductor. This conductor will be bonded to the panelboard enclosure or ground bar, the raceway if necessary, any other enclosures or boxes, including the outlet box for the IG receptacle.
