When do you not have to pull a grounded(green/bare) wire? Only if the entire run is in metallic conduit?

The grounded conductor is the neutral.
The equipment grounding conductor is what you are referring to. Its commonly called a ground, but please Art 100 definition of ground.

A green or bare wire would be a grounding conductor where as the ending "ed" is reserved for the neutral conductor.
