Extending resi service conductors?

So far, when doing residential panel changes, I've been able to avoid trouble with the service conductor lengths: They have either been long enough to reach the mains and the neutral bar on the new panel, or the run between the meter and the panel was short enough where we would just re-feed a longer set of conductors without trouble.
I have seen services, however, where there is 30, 40 feet (sometimes more) between the can and the panel.
If those come up short, we'll have to extend them, which could be costly to the HO if those conductors are copper and the pipe is too small for a set of new AL ones..

So, now with the question:

I have never seen anything in NEC forbidding extending service conductors inside a panel, as long as the gauge of the extra length and the splicing method is appropriate.

That said, have any of you ever run into any problems with AHJ over this?
And if so, what was AHJ's issue with that -- the fact itself, the method of splicing, etc.?
