Maybe they did attempt to remove it and the conductors were so short the device only pulled out a quarter inch max. Plan B was then followed.

You have to love plan "B". I would never have even thought of it. I guess that thinking outside the box.

How would that device be able to be installed in the first place?

Ran across some old armor cable, with the rubber covered wires, feeding an attic j-box yesterday. 90 years worth of heat and age made the insulation very brittle, and the original taped splice a tarry mess. Didnt really want to open it up for fear it would turn into a cluster, but I prevailed. Maybe the person who installed this outlet-over-outlet had the same thing once, and decided not to open up a hornets nest again. Not saying it's right by any means, but I can see one way this could have happened.

Kinda surprised looking at the pics again that the wall wasnt cut back and a cover plate put on the original receptacle, leaving a recessed design. They probably wanted it to look good at the expense of code and safety issues. Reminds me of the saying "Work it may, but shine it must".

I have tried to remove ones that were so close I would swear the installer wired the receptacle and then pulled the cable back out tight.
Seen many myself that had me thinking about the same thing. They are even worse when there is an in and out cable in the box.

I think that many times when the conductors are fished then all the slack is pulled back. I'm not sure they intend to do this, probably just not thinking. I don't make up the device until I have ran the conductors or at least I have the conductors secured (stapled) at a point to make sure the slack will not get pulled back. Also I use deep old works boxes that allow the extra length to be folded back easily. I may have a little more that required length.

I was told a story of an EC from the area that, decades ago, would install receptacles and after they were installed, go below and pull the cable tight. I was also told that when he retired he bought a motorhome with scrap wire he had been hoarding.

I think I have worked on a couple of his jobs.
