Are you referring to a through box where it extends from one side to the other. you can see through it to the other room.
No No pad necessary as you cannot have such a box in a fire rated wall in most instances.
Back to back boxes will need pad on one box at least depending on the rating of the wall.

Metal boxes are permitted in 2 hour rated walls when separated by 24" on opposite sides of the wall without puddy pads.

And another good thing about that link, it convinced my inspector that I didn't need to get the putty pad between the box and the stud. A good inspector who doesn't hesitate to be educated.
