Could someone explain the purpose of this requirement? I didn't think you needed to disconnect/switch grounded conductors, and if you did, the disconnect would have to disconnect the ungrounded and grounded simultaneously.

225.38
(C) Disconnection of Grounded Conductor. Where the
building or structure disconnecting means does not disconnect
the grounded conductor from the grounded conductors
in the building or structure wiring,
other means shall be provided
for this purpose at the location of disconnecting
means
. A terminal or bus to which all grounded conductors
can be attached by means of pressure connectors shall be
permitted for this purpose.
In a multisection switchboard, disconnects for the
grounded conductor shall be permitted to be in any section
of the switchboard, provided any such switchboard section
is marked.

It is so you don't just pull a neutral straight through from the weather head to the distribution panel without a lug or terminal in the main disconnect, so if needed you can remove or service it.

Thanks, never had a reason to even notice this, much less understand what it was saying. I guess this is normally applying to the meter socket, yes?

It applies to all building and service disconnects, not to meter sockets. I have never seen an installation that did not automatically meet this rule.
